Accountable care organisations (ACOs) are under active development in a number of areas of England. They are designed to bring together providers of mental, physical, hospital and community services to provide joined-up care for a population.
Montefiore Health System in New York, is a pioneer for the accountable care organisation model. Working in this way they’ve successfully been able to provide patients with higher quality care, while reducing expenditures through enhanced care coordination.
In this online event we heard from Stephen Rosenthal, Chief Operating Officer at Montefiore Health System. Stephen spoke about how they implemented their ACO model and provide valuable lessons for us in England as we start to develop sustainability and transformation partnerships into accountable care organisations and systems.
We also heard from Herb Fillmore, Senior Director, Market Development, Populations and Payment Solutions at 3M. Herb shared how the data system underpinning their ACO model has enabled Montefiore to deliver their success.
