There has been growing interest in intermediate care as a way of supporting people moving between acute, primary and social care.

But there is a lack of consensus about the nature of these services, how they should be developed, and whether they should be instead of, or in additional to, traditional services.

This publication, funded by the Department of Health, describes seven different models of intermediate care services, highlighting how different organisations, from different parts of the United Kingdom, manage the interface between acute, primary and social care.
