The programme provides an important opportunity to learn how local areas can use major new housing developments as an opportunity to promote population health and deliver integrated care. Completed project A vision for the future of community-based health services Our work to develop a vision for the future of community-based health services. Completed project Measuring the performance of local health systems In June 2015, The King's Fund was commissioned by the Department of Health to review how the performance of local health systems could be assessed. Completed project Older people’s experiences of district nursing services This project was done to improve our understanding of quality in district nursing services by learning from the experiences of older people receiving care, their informal carers and district nursing staff. Completed project The evolution of clinical commissioning: learning from local experience We worked with the Nuffield Trust to understand the development of CCGs and the challenges they face. Completed project Quality in community health services The project explored how providers of community health services in England are monitoring, managing and improving the quality of their services. Completed project Specialists working in out-of-hospital settings This project looked at new models of service provision in which specialists deliver, or facilitate the delivery of, care outside of the traditional hospital setting.
