This live online event will explore how population health management and data-driven insights can be used to inform the planning and delivery of care and improve health and wellbeing outcomes.

Our speakers will discuss the importance of population health management and what success looks like, and share learning on segmentation and stratification techniques to identify groups of patients at risk of ill health. The discussion will explore how to use data effectively to design targeted interventions aimed at preventing ill health or equipping patients to manage it.

They will also explore how integrated care systems can use these techniques to allocate resources, understand their populations and help inform local decision-making and partnership working.
