This live online event explored how population health management and data-driven insights can be used to inform the planning and delivery of care and improve health and wellbeing outcomes.

Our panel provided insight on building collaborative partnerships between the NHS, local government and other partners and the importance of sharing patient information to improve health care service provision and reduce costs.

Our speakers discussed the importance of population health management and what success looks like, and shared learning on segmentation and stratification techniques to identify groups of patients at risk of ill health. The discussion explored how to use data effectively to design targeted interventions aimed at preventing ill health or equipping patients to manage it.

They also explored how integrated care systems can use these techniques to allocate resources, understand their populations and help inform local decision-making and partnership working.
