About this event

Following the NHS five year forward view, new models of care are being trialled across England both by vanguards and other NHS organisations.

This one-day event looked at these models of care in detail. One year on from the launch of the PACS and MCP vanguard sites, it provided an opportunity to hear from the sites and others on how are they working in practice with lessons for areas of the United Kingdom that are interested in implementing similar models for their local populations.
