Sian Jones, Head of Service Improvement at Bristol Clinical Commissioning Group, explains how NHS Bristol provides telehealth monitoring to patients with chronic conditions.

The Safe Mobile Care system enables patients with chronic obstructive pulmonary disease (COPD) and congestive heart disease to benefit from daily remote clinical monitoring using mobile phones. If a response indicates that a patient’s condition may be worsening, an alert is generated automatically and sent to a nurse or doctor who advises the patient on the action that should be taken.
