Robyn Doran, Chief Operating Officer, Central and North West London NHS Foundation Trust, and Dr Muhammed Ali, Clinical Director, Outer North West London Integrated Care Programme, explain how care is being integrated across the boroughs in North West London.
Local people will be supported by GPs who will work with community practitioners to help residents remain independent. People will be given a single point of contact who will work with them to plan all aspects of their care, taking into account all physical, mental and social care needs.
North West London was awarded integrated care pioneer status by the government in November 2013 – one of 14 sites in the country.
