well presented-personally it is vital to work as a team and incentivised to earn money through service delivery and not handing over global sums as happens now to the GPs
the hard workers now get the money after working hard but there are practices who also get the money without working at all

QOF is one good example where the money is hardly invested into the infrastructure but it makes fat cats more fat cats
