Many thanks for your interest. The case study research is ongoing and learning resources on the Sandwell approach will be available on these web pages in the late Spring/early summer 2013 with a final report examining the issues related to care co-ordination to people with complex needs following that later in the year.

Are you able to share details of your service with me? I am assissting in a literature review informing the design of such a service to be developed over 4 years in Tasmania, Australia.

Couldn't find the report in publications etc, is there any update on when it will be published? In the process of developing a strategy and wanting to make reference to their primary care - led model.
