Dr Adrian Treloar explains how Greenwich and Bexley Advanced Dementia Services provide holistic and palliative care at home for people with advanced dementia. The benefits of using this co-ordinated care approach include making savings of £300,000 per year by avoiding hospital and care home admissions.

These services are one of five demonstrator sites we are working with on our co-ordinated care for people with complex chronic conditions project.

Are details of the service Dr Treloar is describing available? In Tasmania we are preparing to roll out a service with the same aims across the island and I am assisting with a literature review. Your information would be very valuable to that review.
