The NHS is developing case management – providing intensive, personally tailored care to those at greatest risk of hospital admission – to help improve care for patients with long-term conditions. This paper reviews the published literature on case management and asks to what extent this approach is benefiting older people. It includes detailed research findings and discussion in the context of current NHS policy, and makes recommendations for more effective case management in the future.
