Assertive outreach is an effective method for reaching and helping people with severe and long-term mental health problems who fall out of contact with local health services.

This publication presents key learning points from the experiences of three community mental health care teams set up in some of London's most deprived communities to promote social inclusion and help people find the right kind of treatment and care.

The projects were set up through Working Together, a £2 million project supported by the Department of Health, The King's Fund and the Sainsbury Centre for Mental Health.

Reaching and supporting Londoners with severe mental health problems identifies and analyses the lessons learned for health and social care professionals planning to meet the requirements of the NHS Plan and offers guidelines for putting policy into practice.
