Intermediate Care Co-ordination cover In 2001, the Department of Health advised the NHS and councils to appoint intermediate care co-ordinators. In response, the Department of Health and the King’s Fund held two workshops attended by early post-holders and other stakeholders. This paper details the results of their discussions about the functions and tasks that each care community should carry out to ensure an effective intermediate care system. It aims to help stakeholders decide the most appropriate way to manage the intermediate care co-ordinator role to suit their needs.
