Reviewing the evidence on case management: lessons for successful implementation

Case management is a key tool in the delivery of integrated care and has become an internationally accepted model of providing intensive care support to people with long-term and complex health and social care needs.
It takes many forms, but primarily represents a targeted, community-based and pro-active approach to care that involves case finding, assessment, care planning, and care co-ordination.
