Evidence submission: House of Lords Committee on Public Service and Demographic Change

This supplementary note on the health and care system in 2025 was submitted following appearances by Chris Ham and Richard Humphries before this committee on behalf of the Fund.

The evidence outlines our preferred vision for the health and care system of 2025. It describes changing population needs as the starting point for a new model of care which is better co-ordinated, more focused on prevention and accompanied by a shift of resources away from acute hospitals to providing care in and closer to home.

Key elements of the model are explored, which also include: reduced variation in the quality of primary care; further concentration of specialist services in centres of excellence supported by networks and an enhanced role for individuals, carers and families to make choices about their care and play a central role in looking after themselves.
