It is a fundamental right that people should be able choose to die at home with dignity – a right that people with dementia do not currently have.
This project will employ a Dementia End of Life Care Nurse to work across the Dementia Services teams and Extra Care Courts in London to provide specialist nursing assessment, interventions, advice and support for service users in their own homes, their carers and families. This project will employ a Dementia End of Life Care Nurse to work across the Dementia Services teams and Extra Care Courts in London to provide specialist nursing assessment, interventions, advice and support for service users in their own homes, their carers and families. The nurse will primarily:
- act as an expert practitioner, advocate and facilitator to meet multiple and often complex needs
- ensure effective co-ordination and engagement of statutory and voluntary services
- provide support, clinical advice, education and training to Housing 21's care and housing staff.
