End-stage Heart Failure: An integrated support package for carers

This project aims to provide a single point of contact of care for patients with end-stage heart failure and their carers. The project care co-ordinator discusses the needs of the patient and their carer, provides support with care planning and acts as a single point of entry to health and welfare services.
