Co-ordinated care for people with complex chronic conditions

Aetna Foundation development day
Date:
Venue:
The King's Fund

About this event

At our development day, we brought together the service leads from the five demonstrator sites we are working with on our co-ordinated care project, to look at the barriers and facilitators to delivering successful joined-up care.

The project involves an in-depth examination of approaches undertaken in primary care settings in different parts of the UK.
