Care co-ordination demonstrator site: South Devon and Torbay
Solveig Sansom explains more about South Devon and Torbay's predictive model and virtual ward. The aim of pro-active case management in South Devon has been to utilise predictive models to identify and support people with a high risk of hospitalisation. The approach is now being introduced into the already successful integrated model of care to older people operating in Torbay.
These services are one of five demonstrator sites we are working with on our co-ordinated care for people with complex chronic conditions project.
