Care co-ordination demonstrator site: Sandwell Integrated Primary Care Mental Health and Wellbeing Service
Lisa Hill tells us more about Sandwell's integrated community service. The service uses care co-ordinators who work with individuals with a range of complex needs, such as those with severe mental illness, to help them navigate and access a wide variety of primary care-based mental health and wellbeing services that support their needs.
These services are one of five demonstrator sites we are working with on our co-ordinated care for people with complex chronic conditions project.

Many thanks for your interest. The case study research is ongoing and learning resources on the Sandwell approach will be available on these web pages in the late Spring/early summer 2013 with a final report examining the issues related to care co-ordination to people with complex needs following that later in the year.
Are you able to share details of your service with me? I am assissting in a literature review informing the design of such a service to be developed over 4 years in Tasmania, Australia.
