Care co-ordination demonstrator site: Midhurst Macmillan Service
Jo Stuttaford and Sue Dewar share how Midhurst Macmillan Service provides palliative care in the community via a consultant-led multi disciplinary team. The team gives 24/7 care and advice for patients in their homes or other preferred place of residence.
This service is one of five demonstrator sites we are working with on our co-ordinated care for people with complex chronic conditions project.
