Care co-ordination demonstrator site: Greenwich and Bexley Advanced Dementia Services
Dr Adrian Treloar explains how Greenwich and Bexley Advanced Dementia Services provide holistic and palliative care at home for people with advanced dementia. The benefits of using this co-ordinated care approach include making savings of £300,000 per year by avoiding hospital and care home admissions.
These services are one of five demonstrator sites we are working with on our co-ordinated care for people with complex chronic conditions project.
